Hello this is being submitted on behalf of the Former Wife by her attorney Mayra Talarico. Attached please find the Final Judgment and MSA regarding the above mentioned matter. The Former Husband Daniel Renteria Villalobos was supposed to contact your office several months ago to begin the QDRO process of his 401K. See Sections 5.1 and 5.2 of the MSA. I have attached the latest statements to said 401k as well. Has he engaged your office yet? If not, kindly copy us all on your instructions and with the forms needed to being the QDRO process. You can send me all correspondence for the Former Wife Margarita Hernandez Baltazar and we will promptly respond to the same.
